ETHICAL APPROVAL

This Ethical approval is valid for——–month only. Any change/s in the protocol or extension in this period of study should be notified to the committee for prior approval.

This ethical approval is issued subject to the following conditions:-

• A signed personal declaration of responsibility
• If the research question/title changes significantly so as to after the nature of study, a new application for ethical clearance must be submitted.
• Principal investigator will inform in writing about progression of Research work at regular 2 months interval
• Principal investigator to submit complete research work to Committee before publishing/sharing/dissemination for review & feedback
• It remains the principal investigator’s responsibility to ensure that all necessary document and consent forms related to the study are retained for a maximum period of tow year for future reference.
